Surrogacy is a method of reproduction whereby a woman agrees to become pregnant and deliver a child for a contracted party. She may be the child's genetic mother (the more traditional form of surrogacy), or she may, as a gestational carrier, carry the pregnancy to delivery after having been implanted with an embryo, the latter being an illegal medical procedure in some jurisdictions.

Surrogacy or Surrogate means substitute. In medical parlance, the term surrogacy means using of a substitute mother in the place of the natural mother.

In traditional surrogacy (also known as the Straight method) the surrogate is pregnant with her biological child, but this child was conceived to relinquish the child to be raised by others such as the biological father and possibly his spouse or partner. The child may be conceived via sexual intercourse (NI), home artificial insemination using fresh or frozen sperm or impregnated via IUI (intrauterine insemination), or ICI (intracervical insemination) which is performed at a fertility

In gestational surrogacy (aka the Host method) the surrogate becomes pregnant via embryo transfer with a child of which she is not the biological mother. She may have arranged to relinquish it to the biological mother or father to raise, or to a parent who is unrelated to the child (e. g. because the child was conceived using egg donation, sperm donation or is the result of a donated embryo). The surrogate mother may be called the gestational carrier.

Altruistic surrogacy is a situation where the surrogate receives no financial reward for her pregnancy or the relinquishment of the child (although usually all expenses related to the pregnancy and birth are paid by the intended parents such as medical expenses, maternity clothing, and other related expenses).

  • Women whose ovaries are producing eggs but they do not have uterus this could be because they have had a hysterectomy (removal of uterus) performed due to cancer, severe hemorrhage or ruptured womb, or they were born without a uterus. This is by far the most common indication for IVF surrogacy.
  • A woman whose uterus is malformed or damaged and is incapable of carrying a pregnancy to term, may be recommended IVF surrogacy.
  • Women who suffer from medical problems such as diabetes, heart and kidney diseases and in whom a pregnancy would be life threatening. However, their long term prospect for health is good.
  • Repeated miscarriages where the causes of miscarriage have been fully investigated, may also suggest IVF surrogacy treatment.

Traditional Surrogacy Treatment in India

  • Women who have no functioning ovaries due to premature menopause ( some may argue that the best option for these patients is egg donation).
  • A woman who is at risk of passing on a genetic disease to her offspring, may opt for traditional surrogacy.
  • As with IVF surrogacy, women who suffer from medical problems such as diabetes, heart and kidney diseases and in whom a pregnancy would be life threatening may select traditional surrogacy if their long term prospect for health is good.

In child bearing, the man’s role is limited to his contribution of the sperm. The patriarchal society is built on man’s protection of his position in the family so that he can be assured that his name will be continued and property will pass to his heirs.

In surrogacy, one man purchases the rights of another. Some men react to this ‘encroachment’ on their rights. Women who participate in surrogacy programmes report that their partners, initially agreeable to their undertaking the responsibility, often change their attitude after they take on their new role.

Children, too, become victims. Some are teased and tormented by friends. Others have shown shame at what their mothers did. Surrogates have to carefully consider how they will explain the pregnancy to their existing children, especially if they are young, and ensure that there is no resentment.
